e2d96f942f Provide nice error output when plugin initialization fails 
Include extra logging during initialization to display error if encountered
and solution set prior to performing activations.

Give a unique, prefixed name to all tempfiles 
This commit basically grepped the code base for all uses of Dir.mktmpdir
and Tempfile.new/open and ensures the value is unique within the
code base and also prefixed with `vagrant-`.

Previously, most invocations of these commands simply used "vagrant",
thus making them indistinguishable when trying to identify leaks.

5b910c186c Fix the resolution of SafeEnv. 
Recent change broke bundler.rb due to incorrect scope resolution (the
error is: uninitialized constant Vagrant::Bundler::SafeEnv
(NameError).

36e924b73e add support for bundler >= 1.6.x 
bundler changed their API for Bundler::UI in 1.6.0 via this commit:

  https://github.com/bundler/bundler/commit/5e38e2d7

This caused various issues, e.g.

  https://github.com/mitchellh/vagrant/issues/3451
  https://github.com/test-kitchen/test-kitchen/issues/401

which will hopefully be resolved by teaching vagrant to cope with both
API versions.

2c72c32df0 core: Modify Bundler settings to work with restricted permissions 
This includes two changes:

1. Change the Vagrant requirement to use the "= VERSION" constraint.
   The "path:" previously requires write permission in the directory
   of the gemspec, which we don't have in installers.

2. Set a BUNDLE_APP_CONFIG and BUNDLE_PATH setting. By setting this,
   Bundler won't ask for sudo password. Bundler never actually needs it;
   it is just overly aggressive in thinking it does.
